Use this service to request a medical report.
Medical report requests can take 28 days to complete. Our full and computer medical records are provided on a password protected email. Brief summaries and small information requests can be sent by email, text message or printed.
For blood and immunisation records for patients over 16, please apply for online services.
You can use this service if you:
- are registered at the surgery
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
