This policy outlines the services that are not included in the core General Medical Services (GMS) contract and will not be provided by New Collegiate Medical Centre, either as an NHS or private service. Registration with the practice constitutes acceptance of these terms.
Assessment of Capacity
We do not assess capacity for legal documents or decisions (e.g., wills, lasting power of attorney)
Dental Care
We do not provide any form of dental care, including prescribing antibiotics for potential infections or converting private dental prescriptions to NHS prescriptions.
Firearms Certificates
We do not assess or provide certificates for firearm licensing.
Transgender Hormone Prescriptions
We do not issue prescriptions for transgender hormone therapy.
Letters for Accessibility or Travel
We do not provide letters for theme parks, holidays, travel, or queuing exemptions. We can provide a patient summary of medical conditions and medications for a fee.
Court Attendance Letters
We do not provide letters stating unfitness for court attendance unless legally mandated in writing by a court official.
Vaccination Exemptions
We do not provide letters stating exemption from vaccinations unless there is documented anaphylaxis with no alternative vaccination available.
Fitness-to-do Letters
We do not provide letters stating fitness to engage in leisure, sporting, charity, educational, occupational, health, or legal activities.
Altitude Sickness Prescriptions
We do not prescribe medications to prevent or treat altitude sickness.
Private Health Checks/Screening
Any tests arranged by private providers will be returned to the patient without clinical review or action and will not be entered into NHS records. The requesting provider remains responsible for follow-up care.
Sedation/Anxiolytics for Phobias
We do not provide sedation or anxiolytic medication for specific fears or phobias, including medical procedures, dentist visits, or flying.
Tests Requested by Private Providers
We do not request, arrange, or perform tests at the request of private healthcare providers unless they are tests, we would normally undertake on behalf of an NHS specialist, as determined by the GP.
Seatbelt Exemption Certificates
We do not provide exemptions for seatbelt use.
Shared Care Agreements
We do not participate in shared care agreements with private providers. For patients using NHS Right to Choose, we may consider shared care, subject to practice resource availability and acceptable terms. There is no guarantee of acceptance, and agreements may be terminated at any time.
Sick Notes for Schools and Universities
We do not provide sick notes for educational institutions.
Weight Loss Medications
We do not assess suitability for weight loss medications prescribed by private providers. Letters regarding these medications will not be reviewed, and a standard letter will be sent to the prescriber advising them of their duty to ensure safe prescribing. Specific information may be provided upon patient consent and payment of the relevant fee.
Weight Loss Surgery Follow-up
We do not provide monitoring services for the first two years post-weight loss surgery. If the surgery was conducted within the NHS, follow-up is the responsibility of the NHS team. If the surgery was private or abroad, the patient must arrange private follow-up care for the first two years.
Passports
As a practice, we do not countersign passport applications and photos.
